Clinical Utilities
C-ANCA are present in 70-90% of patients with generalized Wegener granulomatosis (WG). C-ANCA-postive sera containing PR-3 Ab are 80% sensitive and 97% specific for WG. Positive ANCA results obtained in patients with other types of vasculitis typically give a P-ANCA pattern and are usually associated with autoantibodies to myeloperoxidase. Autoantibodies to bactericidal permeablility increasing protein (BPI) are found in about 91% of cystic fibrosis patients, 28% of ulcerative colitis, 22% of Crohn disease, 20% of primary biliary cirrhosis, 18% of primary sclerosing cholangitis and 5% of Henoch-Schoenlein purpura.
