Warfarin is an anti-coagulant widely used for the prevention of thromboembolic events, but it is associated with about 29,000 emergency department visits per year for bleeding complications. It has a particularly narrow therapeutic window where for one individual, a too-high dose, which can bring about hemorrhage, is not very different from a too-low dose, which lead to thrombotic episodes. Furthermore, the correct dose varies widely between individuals – by as much as 150 fold. It can take many months to achieve stable dosing, with patients at risk for excessive bleeding or ineffective clot control during that time
