Human gongylonemiasis, caused by the nematode Gongylonema spp., of which there are several species (including G. pulchrum) is characterized by a sensation of worm-like migration in the mouth.1-4 Some patients remove the worm(s) themselves. Insects are the intermediate host and intentional or accidental entomophagy (eating insects) is a risk factor even for the fastidious, because the amount of insect components allowed in various foods (e.g., peanut butter, pepper and raisins) is regulated by the FDA.5 Raw or uncooked fish and contaminated water are also sources.6
Differential diagnosis of buccal lesions should include Wuchereria bancrofti6 and Dirofilaria immitis.7
